同名ケータイ小説をゲーム化した恋愛アドベンチャーノベル。プレイヤーは、主人公の"芽衣"となって赤い糸の相手を探すことになる。本作は、DS本体を縦にして片手で持ち、タッチペンや十字キーの下を使ってまるで小説本を読むような感覚でプレイできる。ストーリーは、「赤い糸 上・下」の内容に沿ったもので、中学2年生から高校1年生までの期間が描かれている。各ポイントで選択肢を選ぶことでストーリーは展開。クラス替えや修学旅行、高校受験といったイベントを重ねることで、最終的にはさまざまなエンディングに分岐する。感情移入しやすいように登場人物は"芽衣"を除いてフルボイスとなっている。
